I use these:

They have a left hand spiral, they bite harder the more you tighten and twist the dowel right out. Assortment of sizes, hasn't let me down yet. To remove the extractor from the dowel I clamp the dowel in a plastic attachment for my bench vise to hold them in place.

you can also grab the exposed portion with a small pair of vise grips and then use a pry bar under the v/grips or tap them upward with a hammer and they will usually come right out...sometimes I can just pull them out

Inserting a bolt will also help this technique, to minimize the crushing of the dowel.
