Yes 18 yr old bike i picked up, thought i'd check cam chest & measure runout for gear drive,
found this, anyone seen this occur before?

That looks like a lifter roller failure. Good time to replace inner cam bearings, change to a better cam set and most importantly replace the lifters.

Getting all Replaced ,but lifters in good condition, see pic bottom right lifter was on the worse lobe !
Well, hard to say what caused the wear, the exhaust cam lobe of the rear cam shows some wear as well, could be a bad heat treatment during the final production process. I would also replace the lifters for peace of mind.

I saw that on a set of S&S 570's. I attributed it to a mistake during the case hardening process.
why gear drive ?
It is hard to tell from pictures but yours look good compared to the stock set I pulled out of my bike with about 70K on them. One of the lobes was worn so badly you could easily feel the grove with your fingers. I heard lots of the early stock had hardening issues. And yes, whatever you put back in replace the lifters too. Cheap insurance at this point.

We had witnessed soft cams in that era.....in a Softail also.
Intact roller hogged into the lobe, bike had 1200 miles.
Cam must have missed heat-treat.
