96 Dyna wide Glide Primary Housing bolts loose

96 Dyna Wide Glide the two inner primary housing bolts that fasten the housing to the motor coming loose.  Owner claims thaat the locking tabs were folded over the hex and the the bolts were properly torqued when installed and were the proper length.  The primary was converted some time in the past to a belt drive.

Previous owner had installed heli-coils. Coils pulled out, replacing with "Time-Serts".
